Resources for Clinical Instructors and Center Coordinators of Clinical Education

Our partners in clinical education provide welcoming, enriching, and supportive environments that challenge and prepare our students for independent practice. We greatly appreciate the time and effort our clinical education instructors invest into mentoring our physical therapy students. Our clinical partners are a vital part of our physical therapy program!

Below are forms and information that may assist you in partnering as a clinical site and clinical instructor. Students are responsible for providing you with all required forms, but we encourage you to select any of the additional forms listed below that you feel will enhance the clinical experience.

William Carey DPT Curriculum

Curriculum provides a chart of the classroom curriculum and timing of courses.

Course Descriptions provide course content.

Experiential Learning I Expectations, Experiential Learning II Expectations, Experiential III Expectations, and Experiential Learning IV Expectations provide didactic preparedness to date, objectives, and level of supervision needed for students during each clinical experience.

Assessment Forms

Self-Assessment for Clinical Instructors is provided for CI’s who wish to reflect on their clinical instruction.

Assessment of Director of Clinical Education is provided for the evaluation of William Carey’s Director of Clinical Education on communication, professional behavior, program development, management, and collaboration.

The Clinical Performance Instrument for evaluation of students is done on-line. The Quick Start Guide for the APTA Learning Center is a guide to a short on-line course from the APTA that is required (but free) before you can access the on-line CPI. Once you have completed this review of the CPI, PT CPI Web 2.0 Instructions for a CI will help you log in and access the on-line CPI. If you prefer to use the paper copy of the CPI, scan and email to the DCE at jprehn@wmcarey.edu.

Professional Behavior Assessment Form is used by students to self-assess their professional behavior, with CI input.

Clinical Education Anecdotal Record is used to document a significant incident, either positive or negative, that involved the student. This is useful in supporting scores given by the CI, especially when documenting affective behaviors.

Negotiated Learning Contract is used when the student is in danger of failing the clinical experience. This contract is made in partnership with the school’s Director of Clinical Education, and explicitly states the clinical behaviors that the student should exhibit for the remainder of the experience.

Critical Incident Report can be used to report a series of problem behaviors that places the student in danger of failing the experience

Clinical Feedback Form allows you to give your thoughts and suggestions to the faculty at Carey regarding the curriculum that prepares students for the clinical experience and the readiness of the students for the clinical experience. We value your input.

               Student Preparation

Weekly Planning Form provides an opportunity for students and CI’s to review the past week and set goals for the upcoming week.

Weekly Review Form is suggested as a more specific guide for student reflection, weekly planning, and a means for eliciting feedback from students regarding their perception of the clinical instruction and clinical environment.

Weekly Planning Form B is an alternative weekly reflection and planning form that may be used if preferred by the CI.

Physical Therapist Student Learning Style Questionnaire is used with permission of Johns Hopkins and provides an opportunity for the student to express the ways they are most supported in the clinical learning process.

Clinical Skills Questionnaire is used by permission of Johns Hopkins and provides information on the comfort level of the students for a sampling of clinical activities.

Additional Resources

Reference Manual for Center Coordinators of Clinical Education is a reference provided by the APTA with instructions and suggestions for setting up a clinical education affiliation at your facility.

Clinical Site Information Form is provided by the APTA and may be filled out to provide additional information regarding your facility and clinical instructors. If filled out online, this information can be accessed by other schools with whom you have a contract.

 CSIF Web Instructions for a CCCE On-line will allow your facility information to be viewed by other schools with whom you have a contract.

Minimum Required Skills of Physical Therapist Graduates at Entry-Level may answer questions as to the expected performance and knowledge level of your students.

Documentation Standards can be used with students who need additional feedback and work towards improved documentation.

Additional Clinical Education Resources from the APTA may answer any questions you may have concerning clinical teaching.

Thank you for partnering with William Carey DPT! Your role in our physical therapist education program is essential. We are always interested in your feedback on the readiness of the students, the timing of the clinical education experiences, apparent weaknesses and strengths of the course work and lab sessions, and suggestions for improving our program.
